This notice is to inform you that Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, for the 2025 tax year is available upon request.
Form 1095-C provides information about the health care coverage offered to you by Job World, Inc. during the 2025 calendar year.
You are not required to obtain a copy of Form 1095-C in order to file your federal income tax return; however, you may request a copy for your records.
To request a copy of your Form 1095-C, please submit your request using one of the following methods:

Requests may be submitted at any time, and a copy will be provided within 30 days of receipt of your request.
